It's literally just based on the stats of the stuff. Pearlescent end shelves = 3 eterna, 5 quanta, 7.5 arcana. 14x of those = 40 eterna, 70 quanta, and 105% arcana (I.e. max) End-fused shelf of rectification = 20% rectification. x5 = 100%. Draconic End shelf = 5 eterna. Since there's already 40, and 50 is the max, 2x = 50 (i.e. max) eterna.
No, max is still 30. And im pretty sure those top enchants like advanced prot and supreme sharpness can't be found in the enchantment table. You need to explore for those things. And you can only enchant within their own set. So sharpness 4 and sharpness 4 give sharpness 5.
Is it a normal book and not a written/enchanted book? And do you have any mods or datapacks installed (if so, even if it doesn't seem like they would, double check to make sure they aren't altering the recipe).

When enchanting an item with books, you should first enchant it with a single book, then combine two books and enchant the item with the combined book. Then combine 4 books and pairs of two. Then enchant the two pairs in one book, having a result of a 4 enchantment book. And THEN use that book on your item.
